Athlete participation forms were completed and returned to school by the 18th November 2011 accompanied by 2 passport photographs and signed by the students GP. Athlete participation forms can be downloaded from the link below.
Special Olympics events will be taking place for students aged 6 and above during the 2011/2012 school year. The following PDF link will allow parents to download the Athlete Participation Form. Please note that all sections must be filled out and returned to school with the passport photographs and signed by a doctor.
